Abstract

Benidipine hydrochloride (BH), a medication frequently used by the hypertension patients, acts as a calcium channel blocker. However, its effects on the macrophages have not been investigated thus far. Our goal was investigating the effect of the benidipine hydrochloride to modulate the J774.2 murine macrophage cells inflammatory activity. Our results suggest that in the absence of a standard stimulating agent (LPS) BH did not stimulate the macrophages to produce pro-inflammatory IL-12p40, TNF-α, GM-CSF and IL-6 cytokines. However, when BH was administrated to the cells in the presence of LPS as stimulating agent, it reduced the production of these pro-inflammatory cytokines. Therefore, it had anti-inflammatory activity. At the clinical setting this study suggests that BH can be utilized as hypertension drug that can suppress the inflammation associated with it.

A familiar hypertension medication did more than block calcium channels in this experiment: when macrophages were activated with LPS, benidipine hydrochloride reduced several pro-inflammatory cytokines. But without that inflammatory stimulus, it did not activate the cells. Benidipine hydrochloride, or BH, is a medication frequently used by hypertension patients, and it acts as a calcium channel blocker.

Its effects on macrophages had not been investigated. The goal was to investigate the effect of benidipine hydrochloride on the inflammatory activity of J774.2 murine macrophage cells. Without the standard stimulating agent LPS, BH did not stimulate macrophages to produce the pro-inflammatory cytokines IL-12p40, TNF-alpha, GM-CSF, and IL-6.

With LPS present, BH reduced production of these pro-inflammatory cytokines. The study therefore identified anti-inflammatory activity, and suggested that BH can be utilized as a hypertension drug that can suppress associated inflammation.

The study investigated the immunostimulatory and immunomodulatory activities of BH on mammalian macrophages in the absence and presence of LPS as a stimulant, respectively. Knowing BH's possible pro-inflammatory or anti-inflammatory activities is important for its most efficient utilization in a clinical setting.

If BH suppresses inflammation, patients with acute inflammation associated with hypertension should not be prescribed additional anti-inflammatory drugs to prevent unwanted side effects. BH was purchased from Saba Pharmaceuticals Ltd., and J774.2 macrophage cells were plated in 24-well tissue culture plates.

For immunostimulation tests, subtoxic BH concentrations of one, two, five, and ten micrograms per milliliter were added in the absence of LPS. For immunomodulatory activity tests, one microgram per milliliter of LPS was used per well with BH concentrations of one, five, ten, and twenty micrograms per milliliter.

Wells with only LPS served as positive control groups, untreated wells were control groups, and cell viability was assessed using Trypan blue staining. ELISA tests were conducted for cytokine quantification using kits for IL12p40, GM-CSF, TNF-alpha, and IL-6.

The coating and detection antibodies for each cytokine used a dilution ratio of one to one thousand, and the procedure followed the manufacturer's instructions. The procedure included overnight coating and blocking steps, washes, detection antibody incubation, and HRP antibody incubation.

After the final wash, TMB substrate was added, the reactions were halted with sulfuric acid, and absorbance measurements were recorded at four hundred fifty nanometers. To test immunomodulatory activity, BH was given together with LPS to the macrophages.

After twenty-four hours of incubation, the supernatants were analyzed by ELISA for IL-6 levels. Compared with the positive-control groups treated with only LPS, BH produced a substantial and significant decrease in macrophage IL-6 production.

The result was classified as anti-inflammatory immunomodulatory activity. Figure one measures IL-six release from macrophages after twenty-four hours of benipidine treatment, with and without lipopolysaccharide stimulation.

Without lipopolysaccharide, benipidine does not produce the substantial IL-six response seen with the lipopolysaccharide control, indicating no immunostimulatory activity by itself. With lipopolysaccharide, all tested benipidine concentrations show reduced IL-six release relative to the lipopolysaccharide-only group, supporting the authors’ conclusion that benipidine has anti-inflammatory immunomodulatory activity.

IL12p40 ELISA was performed on supernatants from samples treated with BH in the absence of LPS, and BH was not able to stimulate the cells to produce IL12p40. When cells were activated with LPS and treated with BH, BH led to a significant decrease in IL12p40 production compared with the positive-control groups treated with only LPS.

Figure two measures IL12p40 release after twenty-four hours of benipidine treatment, with and without LPS stimulation. Without LPS, benipidine did not stimulate detectable IL12p40 production, while with one microgram per milliliter LPS, all tested benipidine concentrations showed significant differences from the LPS-only positive control.

This matters because it supports the authors’ conclusion that benipidine has anti-inflammatory immunomodulatory activity without independently activating this cytokine response. GM-CSF production was measured when macrophages were treated with different BH concentrations in the absence of LPS, and BH was not able to stimulate GM-CSF production by itself.

When the cells were activated with LPS, BH significantly suppressed GM-CSF production. Figure four measures GM-CSF release from macrophages after twenty-four hours of benipidine treatment, with and without LPS stimulation. Without LPS, benipidine does not stimulate detectable GM-CSF production, while in the LPS-treated panel, the bars and significance markers show reduced GM-CSF release across the tested concentrations compared with the LPS-only positive control.

With three samples per condition, the figure supports the authors’ conclusion that benipidine has anti-inflammatory immunomodulatory activity without an immunostimulatory effect. BH was administered to macrophages in the presence of LPS, mimicking an ongoing inflammation in the body through an in vitro setting.

A substantial and significant decrease was observed in production of the pro-inflammatory cytokines TNF-alpha, IL-6, GM-CSF, and IL12p40. The result implies that, in a clinical setting, hypertension patients with inflammatory reactions can be prescribed BH alone, eliminating the need for additional anti-inflammatory drugs.

This approach not only simplifies treatment but also helps prevent drug overdosage and possible side effects. In this macrophage model, benidipine hydrochloride did not stimulate inflammatory cytokine production on its own and reduced four pro-inflammatory cytokines when LPS was present, suggesting anti-inflammatory activity that may matter for hypertension treatment.
